SUBSTITUTE APPLICATION INSTRUCTIONS

THIS PACKET MUST BE RETURNED IN PERSON.  MAIL-IN APPLICATIONS ARE NOT ACCEPTED.

INSTRUCTIONS

1. Fully complete the DeKalb County Board of Education substitute packet.

2. Submit a complete medical section IF applying to substitute for Custodial or CNP Positions. 

3. Complete the Alabama and Federal Tax Withholding Forms. 

4. Complete the top half of the I-9 form.  This form must be signed by the applicant.

5. Attach a copy of the following documents: (1) Driver’s License, (2) Social Security Card, (3) High School Diploma or GED Certificate, (4) Voided Check

6.Fill out Application for Substitute License and attach a U.S. Postal Money Order or a Cashier’s Check for $38.00 made out to the ALSDE and return with your packet to the payroll department. 

7. Fingerprints must be done thru Fieldprint. There is an applicant fee of $46.20.  Applicants must schedule an appointment with Fieldprint before visiting a fingerprint location. Fee payment is by debit or credit card only.  No money orders or checks are accepted.
